光氣在常溫下為無色氣體,有爛干草或爛蘋果味,難溶于水、易溶于有機溶劑,在高濃度光氣中,中毒者在幾分鐘內由于反射性呼吸、心跳停止而死亡。光氣的學名叫二氯碳酰,由英國化學家首先于1812年合成。由于當時是采用一氧化碳與氯氣在強光照射下合成的,因而得名為“光氣”。在第一次世界大戰中,德軍對英軍首先使用了裝有光氣和氯氣的鋼瓶,開創了毒氣應用于戰場的先河。

制取

實驗室制取時,可用四(si)氯化碳與(yu)發(fa)(fa)煙(yan)(yan)硫酸反(fan)應。將四(si)氯化碳加熱至55-60℃,滴加入(ru)發(fa)(fa)煙(yan)(yan)硫酸,即(ji)發(fa)(fa)生逸出光氣(qi)(qi),如需使用液態光氣(qi)(qi),則將發(fa)(fa)生的光氣(qi)(qi)加以冷凝。

反(fan)應方(fang)程式:SO3+CCl4→SO2Cl2+COCl2↑

工(gong)業上通常采用(yong)一氧化(hua)碳與(yu)氯(lv)氣(qi)(qi)的(de)(de)反(fan)應(ying)(ying)得到光氣(qi)(qi)。這是一個(ge)強烈放熱的(de)(de)反(fan)應(ying)(ying),裝有活性炭的(de)(de)合(he)成器(qi)應(ying)(ying)有水冷卻夾套(tao),控制(zhi)反(fan)應(ying)(ying)溫度200℃左(zuo)右。為了(le)獲得高(gao)質量的(de)(de)光氣(qi)(qi)和(he)減少設備的(de)(de)腐蝕,經過(guo)徹(che)底干燥的(de)(de)一氧化(hua)碳在(zai)與(yu)氯(lv)氣(qi)(qi)混合(he)時,應(ying)(ying)保持適當過(guo)量。將混合(he)氣(qi)(qi)從合(he)成器(qi)上部(bu)通入(ru),經過(guo)活性炭層(ceng)后,很快轉化(hua)為光氣(qi)(qi)。當選用(yong)規格為Ф700×2900的(de)(de)合(he)成器(qi)時,每臺(tai)可年產光氣(qi)(qi)200t。

原料消耗定額:氯氣(>99%)925kg/t;氧氣(標(biao)準狀(zhuang)況下)268(m3)kg/t;焦(jiao)炭(tan)400kg/t。

氯(lv)仿,雙氧水直接反應:

CHCl3+H2O2=HCl+H2O+COCl2(光氣(qi))——氯仿保管不當易被氧(yang)化產生光氣(qi)。

也可用雙氧水(shui)制出氧氣(qi)后與氯仿反應:

2CHCl3+O2=2HCl+2COCl2(光(guang)氣)

光氣泄露后用(yong)水霧吸(xi)收,光氣很容易水解:

COCl2+H2O=2HCl+CO2

也可用氯氣和一(yi)氧化(hua)碳反應(ying)

毒性

概述

本品是(shi)典型的(de)暫時性毒(du)(du)劑(ji)。吸入中毒(du)(du)的(de)半(ban)致命劑(ji)量LD50為3200mg/kg,半(ban)失能劑(ji)量1600mg·min/m3。吸入后,經幾小(xiao)時的(de)潛伏期出現癥(zheng)狀(zhuang),表現為呼吸困難、胸部壓痛、血壓下降,嚴重時昏迷(mi)以至死亡。防毒(du)(du)面具可有(you)效地防護,通常不需消毒(du)(du)。抗毒(du)(du)藥(yao)有(you)烏洛托品等。出現肺水腫(zhong)癥(zheng)狀(zhuang)者禁止人工呼吸。

健康危害

侵入(ru)途徑:吸入(ru)、經皮吸收。健康(kang)危害:主要損(sun)害呼吸道,導致化學性支氣管炎、肺炎、肺水腫。

急(ji)性中毒(du):輕(qing)(qing)度中毒(du),患(huan)者有流淚、畏光、咽部不適、咳(ke)嗽、胸悶等;中度中毒(du),除上述癥狀加重外,患(huan)者出(chu)現輕(qing)(qing)度呼(hu)(hu)吸(xi)困難、輕(qing)(qing)度紫紺;重度中毒(du)出(chu)現肺水腫(zhong)或成(cheng)人呼(hu)(hu)吸(xi)窘迫綜(zong)合征(zheng),患(huan)者劇(ju)烈咳(ke)嗽、咯大量泡沫(mo)痰、呼(hu)(hu)吸(xi)窘迫、明顯紫紺。肺水腫(zhong)發生前有一(yi)(yi)段(duan)時(shi)間的癥狀緩解(jie)期(一(yi)(yi)般1-24小時(shi))。可并發縱隔及(ji)皮下氣腫(zhong)。

毒理與環境

急性毒性:LC50 1400mg/m3,1/2小(xiao)時(大鼠吸(xi)入);人吸(xi)入3200mg/m3,致(zhi)死;人吸(xi)入25ppm×30分鐘(zhong),最小(xiao)致(zhi)死濃度。

亞急性(xing)和慢性(xing)毒性(xing):動物吸入0.0008mg/L,5小時(5天),40%出現肺氣腫。

污染來源:光氣用(yong)作聚氨酯制(zhi)品處理劑、增塑劑、聚碳(tan)酸(suan)酯原(yuan)料,纖維處理劑、除(chu)草劑、炸藥(yao)穩定劑、染料、染料中間體和藥(yao)品原(yuan)料,在生產(chan)中的跑、冒、滴、漏(lou)或意外泄漏(lou)。

危險特(te)性(xing)(xing):不燃。化學反應活(huo)性(xing)(xing)較高,遇水后有強烈(lie)腐蝕性(xing)(xing)。

中毒機理

光氣吸入中毒(du)后的主要病(bing)變是(shi)中毒(du)性肺(fei)水(shui)腫。肺(fei)水(shui)腫是(shi)肺(fei)毛細血(xue)(xue)管滲透性增強的結(jie)果。對(dui)于肺(fei)水(shui)腫產(chan)生的原因(yin),學說頗(po)多(duo),諸(zhu)如酰(xian)化作用(yong)(yong)、直接作用(yong)(yong)、鹽酸作用(yong)(yong)、神(shen)經反射作用(yong)(yong)、肺(fei)血(xue)(xue)流動力學改變等等,各(ge)有實驗依據。但任何(he)一種假說都不能完滿地解釋肺(fei)水(shui)腫發生與發展過(guo)程

一般認(ren)為肺(fei)毛(mao)細(xi)血管(guan)(guan)壁通(tong)透性(xing)(xing)(xing)增(zeng)(zeng)強(qiang)與光(guang)氣(qi)的(de)(de)(de)(de)酰(xian)(xian)化作(zuo)用(acylation)有密(mi)切關(guan)系。光(guang)氣(qi)為酰(xian)(xian)鹵類化合(he)物(wu),活性(xing)(xing)(xing)基(ji)團是(shi)羰(tang)基(ji)(O=C),化學性(xing)(xing)(xing)質(zhi)非(fei)常(chang)活潑(po),它與肺(fei)組織(zhi)蛋白中(zhong)(zhong)的(de)(de)(de)(de)氨基(ji)、巰基(ji)、羥基(ji)等重要(yao)功(gong)能基(ji)團發生(sheng)酰(xian)(xian)化反應,引(yin)起肺(fei)酶系統的(de)(de)(de)(de)廣泛抑制,從而影響細(xi)胞正常(chang)代(dai)謝及其(qi)功(gong)能,使肺(fei)氣(qi)-血屏(ping)障受損,導(dao)致肺(fei)毛(mao)細(xi)血管(guan)(guan)通(tong)透性(xing)(xing)(xing)增(zeng)(zeng)高,引(yin)起肺(fei)水腫。此外,光(guang)氣(qi)中(zhong)(zhong)毒時(shi),肺(fei)泡(pao)(pao)(pao)表(biao)(biao)面(mian)(mian)活性(xing)(xing)(xing)物(wu)質(zhi)受損也是(shi)重要(yao)因素之一。正常(chang)時(shi),在肺(fei)泡(pao)(pao)(pao)表(biao)(biao)面(mian)(mian)覆蓋一層由(you)肺(fei)泡(pao)(pao)(pao)Ⅱ型上皮細(xi)胞分泌出來的(de)(de)(de)(de)表(biao)(biao)面(mian)(mian)活性(xing)(xing)(xing)物(wu)質(zhi),該物(wu)質(zhi)有降低肺(fei)泡(pao)(pao)(pao)內液(ye)體(ti)表(biao)(biao)面(mian)(mian)張力(li)的(de)(de)(de)(de)作(zuo)用,使肺(fei)泡(pao)(pao)(pao)在呼氣(qi)時(shi)不致萎(wei)陷(xian),并保持肺(fei)泡(pao)(pao)(pao)內的(de)(de)(de)(de)干燥。二棕櫚酰(xian)(xian)磷脂(zhi)酰(xian)(xian)膽(dan)堿(dipalmitoyl phosphatidyl choline,DPPC)是(shi)肺(fei)表(biao)(biao)面(mian)(mian)活性(xing)(xing)(xing)物(wu)質(zhi)的(de)(de)(de)(de)主要(yao)成分之一。在其(qi)生(sheng)物(wu)合(he)成中(zhong)(zhong)需(xu)要(yao)脂(zhi)酰(xian)(xian)輔酶A酯酰(xian)(xian)轉移酶的(de)(de)(de)(de)參與。光(guang)氣(qi)中(zhong)(zhong)毒后(hou),該酶活性(xing)(xing)(xing)下降,因而DPPC在肺(fei)泡(pao)(pao)(pao)壁的(de)(de)(de)(de)含量減少,使肺(fei)泡(pao)(pao)(pao)表(biao)(biao)面(mian)(mian)活性(xing)(xing)(xing)物(wu)質(zhi)功(gong)能下降,從而肺(fei)泡(pao)(pao)(pao)內液(ye)體(ti)表(biao)(biao)面(mian)(mian)張力(li)增(zeng)(zeng)大而致肺(fei)泡(pao)(pao)(pao)萎(wei)陷(xian),肺(fei)泡(pao)(pao)(pao)壓明顯降低,與其(qi)相抗衡的(de)(de)(de)(de)肺(fei)毛(mao)細(xi)血管(guan)(guan)流體(ti)靜(jing)力(li)壓就增(zeng)(zeng)高,液(ye)體(ti)由(you)血管(guan)(guan)內大量外滲,導(dao)致肺(fei)水腫的(de)(de)(de)(de)產生(sheng)。

病理變化

光(guang)氣(qi)、雙光(guang)氣(qi)中毒(du)的病理生理改(gai)變(bian)主要(yao)由肺水腫所引起。

吸(xi)入中毒時(shi),先出(chu)現短(duan)暫的呼(hu)吸(xi)變慢,繼(ji)之(zhi)呼(hu)吸(xi)淺而快。在(zai)出(chu)現早期肺水腫后,由于肺泡呼(hu)吸(xi)表面(mian)積減少,肺泡壁增厚,影(ying)響了肺泡內氣體交換。加上水腫液(ye)充(chong)塞呼(hu)吸(xi)道(dao),支氣管痙攣及其粘膜腫脹所引(yin)起的支氣管狹(xia)窄(zhai),造(zao)成(cheng)肺通氣障(zhang)礙,結果出(chu)現呼(hu)吸(xi)性血(xue)缺氧,導致血(xue)氧含(han)量降低,CO2含(han)量增多(duo),皮膚粘膜呈青(qing)紫色。此時(shi)呼(hu)吸(xi)循環(huan)功能有(you)(you)代(dai)償性變化,如(ru)呼(hu)吸(xi)加快、肋(lei)間(jian)肌活動增強、心跳快而有(you)(you)力、血(xue)壓微升等。

肺水腫(zhong)(zhong)晚(wan)期,由于①肺泡(pao)內(nei)含有(you)大(da)量(liang)液(ye)(ye)體(ti)(ti),肺內(nei)壓力(li)(li)(li)增(zeng)加,使(shi)(shi)(shi)右心(xin)負荷(he)增(zeng)加;②血(xue)(xue)漿(jiang)大(da)量(liang)滲入肺內(nei)使(shi)(shi)(shi)血(xue)(xue)循環內(nei)血(xue)(xue)容量(liang)減(jian)少、血(xue)(xue)液(ye)(ye)濃(nong)縮(suo)、粘(zhan)稠度增(zeng)加。外周(zhou)阻力(li)(li)(li)增(zeng)加,使(shi)(shi)(shi)左心(xin)負荷(he)加重;③長時期嚴重的血(xue)(xue)缺氧(yang)使(shi)(shi)(shi)肌營養(yang)不(bu)良(liang),因(yin)此可出(chu)現心(xin)收縮(suo)力(li)(li)(li)減(jian)弱(ruo)、心(xin)律失常、循環減(jian)慢、血(xue)(xue)壓逐(zhu)漸降(jiang)低(di)等心(xin)功能衰(shuai)竭(jie)表(biao)現。后者又(you)可加重組織(zhi)缺氧(yang),體(ti)(ti)內(nei)氧(yang)化不(bu)全產物增(zeng)加,發生(sheng)酸中毒(du)和(he)電解(jie)質紊亂。血(xue)(xue)內(nei)CO2含量(liang)逐(zhu)漸降(jiang)低(di),內(nei)臟毛細(xi)血(xue)(xue)管(guan)擴張(zhang),外周(zhou)毛細(xi)血(xue)(xue)管(guan)收縮(suo),皮膚粘(zhan)膜轉(zhuan)為蒼白(bai),血(xue)(xue)壓急劇下降(jiang),出(chu)現急性循環衰(shuai)竭(jie),進入休克(ke)狀態(tai)。此期因(yin)肺水腫(zhong)(zhong)合并循環衰(shuai)竭(jie),機體(ti)(ti)失去代償(chang)能力(li)(li)(li)。

隨(sui)著肺水腫(zhong)的發展(zhan),血(xue)(xue)(xue)漿從肺毛細血(xue)(xue)(xue)管大量外(wai)滲,造成血(xue)(xue)(xue)漿容量降低,血(xue)(xue)(xue)液濃縮(suo),出現(xian)血(xue)(xue)(xue)漿蛋(dan)白減少,紅(hong)、白細胞數及血(xue)(xue)(xue)紅(hong)蛋(dan)白增(zeng)加,血(xue)(xue)(xue)球(qiu)比積增(zeng)高。這些變化與肺水腫(zhong)程度相一致。由(you)于血(xue)(xue)(xue)液粘稠、血(xue)(xue)(xue)流緩(huan)慢,加上(shang)組織的破(po)壞,使(shi)血(xue)(xue)(xue)液凝固性(xing)增(zeng)加。由(you)此,可形成血(xue)(xue)(xue)栓和栓塞。

中(zhong)樞神經系統對缺(que)氧(yang)很(hen)敏感。缺(que)氧(yang)初期大(da)(da)腦皮質興(xing)奮(fen),出(chu)現(xian)煩躁不安(an)、頭痛、頭暈等(deng);缺(que)氧(yang)嚴重(zhong)時(shi),逐漸轉(zhuan)入(ru)抑(yi)(yi)制,表情(qing)淡(dan)漠,乏力等(deng)。缺(que)氧(yang)進(jin)一步發(fa)展,大(da)(da)腦皮層(ceng)抑(yi)(yi)制加深,并向皮層(ceng)下擴散,呼(hu)吸(xi)、循環(huan)中(zhong)樞可由興(xing)奮(fen)轉(zhuan)為抑(yi)(yi)制,呼(hu)吸(xi)、心(xin)跳減弱(ruo),以至出(chu)現(xian)中(zhong)樞麻痹,導致呼(hu)吸(xi)、心(xin)跳停(ting)止而死亡。

病(bing)(bing)理解(jie)剖可見皮膚蒼白、胸廓擴大、肋間隙消(xiao)失、口鼻(bi)有粉紅色(se)泡沫狀分(fen)泌物排出(chu)(chu),擠壓胸腔時流(liu)出(chu)(chu)更多。肺(fei)(fei)部因肺(fei)(fei)水(shui)腫、肺(fei)(fei)氣腫、肺(fei)(fei)充血、肺(fei)(fei)不張及輕度(du)出(chu)(chu)血等(deng)各種病(bing)(bing)變彌漫相間而呈(cheng)現(xian)“大理石樣”。

臨床表現

光氣(qi)(雙光氣(qi))中(zhong)毒(du)(du),根據中(zhong)毒(du)(du)程度(du)(du)(du),臨(lin)床上可分(fen)輕(qing)度(du)(du)(du)、中(zhong)度(du)(du)(du)、重度(du)(du)(du)及閃(shan)電(dian)(dian)型(xing)四(si)型(xing)。輕(qing)度(du)(du)(du)中(zhong)毒(du)(du)、癥狀(zhuang)很輕(qing),分(fen)期不(bu)明顯,僅表現(xian)為消化不(bu)良和支氣(qi)管炎癥狀(zhuang),一周內(nei)即(ji)可恢復。閃(shan)電(dian)(dian)型(xing)中(zhong)毒(du)(du)極為少見(jian),多發生(sheng)在吸入毒(du)(du)劑濃度(du)(du)(du)極高時,在中(zhong)毒(du)(du)后幾分(fen)鐘(zhong)內(nei),可因反射性呼吸、心(xin)跳停止而(er)死亡。

光氣可以用于(yu)制備活化聚(ju)乙二醇(chun),用于(yu)修飾蛋白藥(yao)物,增加蛋白藥(yao)物的(de)穩定性

應急處理

迅速撤(che)離泄(xie)漏(lou)(lou)(lou)污染區人員至上風處(chu)(chu)(chu),并立即進行隔(ge)離,小泄(xie)漏(lou)(lou)(lou)時隔(ge)離150米,大泄(xie)漏(lou)(lou)(lou)時隔(ge)離450米,嚴格限制出入。建議應急處(chu)(chu)(chu)理(li)人員戴自給正壓式呼(hu)吸(xi)器,穿防毒服(fu)。從上風處(chu)(chu)(chu)進入現場。盡可(ke)能(neng)切斷(duan)泄(xie)漏(lou)(lou)(lou)源(yuan)。合理(li)通風,加速擴散。噴(pen)氨水(shui)或(huo)其它稀堿液中和。構筑(zhu)圍(wei)堤或(huo)挖(wa)坑收容產(chan)生(sheng)的大量廢(fei)水(shui)。漏(lou)(lou)(lou)氣容器要妥善處(chu)(chu)(chu)理(li),修復、檢驗后再用(yong)。

光(guang)(guang)氣(qi)(qi)(qi)很(hen)容易水(shui)解(jie),即使在(zai)冷水(shui)中,光(guang)(guang)氣(qi)(qi)(qi)的水(shui)解(jie)速度(du)也(ye)很(hen)快(kuai)。水(shui)源、含水(shui)食物(wu)(wu)(wu)以及易吸水(shui)的物(wu)(wu)(wu)質均(jun)不會染(ran)毒(du)。光(guang)(guang)氣(qi)(qi)(qi)與氨很(hen)快(kuai)反應,主(zhu)要生成(cheng)(cheng)尿(niao)素和氯化銨等無(wu)毒(du)物(wu)(wu)(wu)質,因此,濃氨水(shui)可(ke)對光(guang)(guang)氣(qi)(qi)(qi)消(xiao)毒(du)。光(guang)(guang)氣(qi)(qi)(qi)與有機胺作用(yong)(yong),生成(cheng)(cheng)二苯脲白(bai)色沉淀和苯胺鹽(yan)酸鹽(yan)。可(ke)用(yong)(yong)此反應來檢驗光(guang)(guang)氣(qi)(qi)(qi)。光(guang)(guang)氣(qi)(qi)(qi)在(zai)堿(jian)溶液(ye)中很(hen)快(kuai)被分解(jie),生成(cheng)(cheng)無(wu)毒(du)物(wu)(wu)(wu)質。各(ge)種堿(jian)、堿(jian)性物(wu)(wu)(wu)質均(jun)可(ke)對光(guang)(guang)氣(qi)(qi)(qi)進行消(xiao)毒(du)。

防護措施

呼(hu)(hu)吸系統防護(hu):正常作業時,應該佩戴(dai)過濾式(shi)防毒面具(全面罩(zhao))。緊急事(shi)態搶救或撤離時,建(jian)議佩戴(dai)空(kong)氣呼(hu)(hu)吸器。眼睛防護(hu):呼(hu)(hu)吸系統防護(hu)中已作防護(hu)。

身(shen)體(ti)防護:穿膠布防毒衣。

手(shou)(shou)防護:戴(dai)橡膠手(shou)(shou)套。

其(qi)它(ta):工作現場禁止吸(xi)煙、進食和(he)飲水。實行就(jiu)業(ye)前和(he)定(ding)期(qi)的體檢。

急救措施

皮(pi)膚(fu)接觸:脫去被污染的衣著,用流動清水沖洗。

眼睛接觸(chu):提起(qi)眼瞼,用流(liu)動(dong)清水(shui)或生理鹽水(shui)沖洗。就醫(yi)。

吸入:迅速脫離現(xian)場至空氣新鮮(xian)處(chu)。保持(chi)呼(hu)吸道通暢。如呼(hu)吸困難,給輸(shu)氧。如呼(hu)吸停止,立即進行人工呼(hu)吸。就(jiu)醫。

滅火(huo)方法:本品(pin)不燃。消防人員必須(xu)佩戴過濾(lv)式(shi)(shi)防毒(du)面(mian)具(全(quan)面(mian)罩)或隔離(li)式(shi)(shi)呼吸器、穿全(quan)身防火(huo)防毒(du)服,在上風處(chu)滅火(huo)。切斷氣(qi)源。噴(pen)水(shui)冷卻容器,可(ke)能的話將(jiang)容器從火(huo)場(chang)移至(zhi)空(kong)曠處(chu)。萬一有光氣(qi)漏逸,微(wei)量時可(ke)用(yong)水(shui)蒸氣(qi)沖散(san),較大時,可(ke)用(yong)液(ye)氨噴(pen)霧沖洗。滅火(huo)劑(ji):霧狀水(shui)、干粉、二氧(yang)化(hua)碳。

中毒分期

中、重度(du)中毒病情發展迅速而嚴重,典型(xing)的臨(lin)床表(biao)現可分為四期:

刺激期

吸(xi)入光氣(qi)立即出現刺(ci)激癥(zheng)(zheng)(zheng)狀(zhuang)(zhuang),主要(yao)表現有:眼(yan)和呼(hu)吸(xi)道(dao)刺(ci)激癥(zheng)(zheng)(zheng)狀(zhuang)(zhuang)出現早,有眼(yan)痛、流淚、咳(ke)嗽、胸悶氣(qi)憋、呼(hu)吸(xi)率改變、嗅(xiu)覺異常或(huo)久存光氣(qi)味,咽喉部及胸骨后疼(teng)痛等;植物神經和中樞神經系統癥(zheng)(zheng)(zheng)狀(zhuang)(zhuang)有頭痛、頭暈(yun)、乏力、不(bu)安(an)或(huo)少言、淡漠、惡(e)心、嘔吐、上腹疼(teng)痛等。在光氣(qi)吸(xi)入劑量相等的情況下,高濃(nong)度短時(shi)(shi)(shi)間中毒(du),刺(ci)激癥(zheng)(zheng)(zheng)狀(zhuang)(zhuang)重;低濃(nong)度長時(shi)(shi)(shi)間中毒(du)刺(ci)激癥(zheng)(zheng)(zheng)狀(zhuang)(zhuang)較輕。但吸(xi)入劑量較大(da)時(shi)(shi)(shi),呼(hu)吸(xi)道(dao)的刺(ci)激癥(zheng)(zheng)(zheng)狀(zhuang)(zhuang)明顯(xian),持續時(shi)(shi)(shi)間也(ye)較長。

潛伏期

刺激癥(zheng)狀(zhuang)消失或減輕,自覺(jue)癥(zheng)狀(zhuang)好轉,但病理(li)過程仍在發(fa)展,肺水腫在逐(zhu)漸形成中(zhong)。潛伏期一般(ban)為(wei)2~13小(xiao)時(shi)。重度(du)中(zhong)毒(du)2~4小(xiao)時(shi),甚至(zhi)(zhi)1小(xiao)時(shi);中(zhong)度(du)中(zhong)毒(du)為(wei)8~12小(xiao)時(shi),有時(shi)長至(zhi)(zhi)24小(xiao)時(shi)。

肺水腫期

從潛(qian)伏(fu)期(qi)到肺水腫期(qi)可(ke)突然發生(sheng)或緩慢發生(sheng),此(ci)期(qi)一般為1~3天(tian)。

肺(fei)(fei)(fei)(fei)水腫(zhong)(zhong)(首先出現間質性(xing)(xing)肺(fei)(fei)(fei)(fei)水腫(zhong)(zhong))的(de)早(zao)期癥(zheng)(zheng)狀(zhuang)有(you):全身疲倦、頭痛、胸(xiong)(xiong)悶、呼吸淺快、脈(mo)搏增加、咳嗽(sou)、煩躁(zao)不安等。聽(ting)診(zhen)呼吸音(yin)減弱(ruo),肺(fei)(fei)(fei)(fei)底部有(you)細濕(shi)性(xing)(xing)啰(luo)音(yin)或捻發音(yin)。胸(xiong)(xiong)部影象(xiang)學檢(jian)查有(you)肺(fei)(fei)(fei)(fei)水腫(zhong)(zhong)征(zheng)象(xiang)。繼之,全身狀(zhuang)況惡化、很(hen)快出現肺(fei)(fei)(fei)(fei)泡(pao)性(xing)(xing)肺(fei)(fei)(fei)(fei)水腫(zhong)(zhong)。典型的(de)癥(zheng)(zheng)狀(zhuang)和(he)體(ti)征(zheng)為(wei)氣喘、呼吸困(kun)難、頻(pin)繁咳嗽(sou)、咯出大量粉紅色泡(pao)沫痰液、脈(mo)快、惡心、嘔吐(tu)及上腹部疼痛等。叩診(zhen)胸(xiong)(xiong)部可聽(ting)到鼓音(yin)及濁音(yin)。肺(fei)(fei)(fei)(fei)下界降低(di),心濁音(yin)界消(xiao)失(shi);聽(ting)診(zhen)時全肺(fei)(fei)(fei)(fei)滿(man)布干性(xing)(xing)及濕(shi)性(xing)(xing)啰(luo)音(yin)。血(xue)(xue)(xue)液檢(jian)查表現為(wei)血(xue)(xue)(xue)液濃縮征(zheng)象(xiang),動(dong)脈(mo)血(xue)(xue)(xue)氧(yang)分壓、血(xue)(xue)(xue)氧(yang)飽和(he)度(du)降低(di)。肺(fei)(fei)(fei)(fei)泡(pao)性(xing)(xing)肺(fei)(fei)(fei)(fei)水腫(zhong)(zhong)進展很(hen)快,一般在24小時內達(da)到高(gao)峰。肺(fei)(fei)(fei)(fei)水腫(zhong)(zhong)期以(yi)循環系統功能是(shi)否良好又(you)可分為(wei)兩個階段(duan)。

1.紫(zi)紺型缺(que)氧階(jie)段(duan):此階(jie)段(duan)血氧含量下降,皮膚粘膜紫(zi)紺、但循(xun)環功能尚能代(dai)償。血壓正常(chang)或稍高,脈搏快而有力(li)。神志清(qing)楚(chu),體溫升(sheng)高可(ke)(ke)達38~39℃。由于肺水腫使CO2排(pai)出(chu)障礙,血中(zhong)碳(tan)酸增高,導致呼(hu)(hu)吸性酸中(zhong)毒;也(ye)可(ke)(ke)因過(guo)度換氣(qi)使CO2排(pai)出(chu)過(guo)多,造(zao)成呼(hu)(hu)吸性堿中(zhong)毒。

2.蒼白(或休克)型(xing)缺氧階段:病情繼(ji)續惡化,呼吸極度(du)困難(nan)。嚴重時全(quan)部呼吸輔助(zhu)肌均參加運動,逐漸出現循環衰竭:脈(mo)細數不規則、血(xue)壓下降(jiang)、皮膚(fu)粘膜(mo)蒼白、出冷(leng)汗、逐漸陷入昏(hun)迷(mi)。此時,血(xue)氧含(han)量(liang)更低,氧化不全(quan)產物增(zeng)加,導致代(dai)謝(xie)性酸中(zhong)毒。

光氣(qi)(雙光氣(qi))中(zhong)毒后的癥狀和體(ti)征24~48小時(shi)達到(dao)高峰,如(ru)不及時(shi)救治,可在1~3天(tian)內(nei)死亡(wang)(wang),重度中(zhong)毒可能在5天(tian)內(nei)死亡(wang)(wang)。因此,凡吸入光氣(qi)者至少(shao)需嚴密觀察(cha)3~4天(tian)。

恢復期

中毒較(jiao)輕(qing)或經(jing)治療后肺水腫液可于發病后2~4天內吸(xi)收,全身情況好轉。咳嗽、氣短減輕(qing)、痰量減少(shao)、體溫(wen)下降,肺部啰音減少(shao)或消失。X線檢查,肺功(gong)能及血氣分析(xi)結(jie)果逐漸(jian)恢(hui)復正常(chang)。一般在中毒后5~7天基本痊愈,2~3周可恢(hui)復健康(kang)。但數周內仍(reng)有頭(tou)暈、咽干、食欲不振、呼吸(xi)循(xun)環功(gong)能不穩定等。

如有繼發感染,一(yi)般(ban)在中毒(du)后第(di)3~4天病情惡化(hua)。體(ti)溫(wen)繼續升高,肺水(shui)腫吸(xi)收遲(chi)緩,可在中毒(du)后8~15天因支氣管(guan)肺炎而死亡。此(ci)外,還(huan)可能發生其它并發癥,如胸膜(mo)炎、支氣管(guan)炎,偶見肺栓塞、肺壞疽、肺膿腫以及下肢(zhi)、腦(nao)、心、視網膜(mo)等處(chu)栓塞。

后遺癥

主要有慢性(xing)支氣(qi)(qi)(qi)管(guan)炎、肺氣(qi)(qi)(qi)腫(zhong)、支氣(qi)(qi)(qi)管(guan)擴張、晚期肺膿腫(zhong)、結核病體質(zhi)等(deng)。光氣(qi)(qi)(qi)中毒的預后(hou)取(qu)決(jue)于(yu)吸入的劑量、病情、救治情況(kuang)及并發癥(zheng)。潛(qian)伏期中難以判斷預后(hou)。出現蒼白型窒息(xi)者多預后(hou)不良。死亡時(shi)(shi)間大多在中毒后(hou)1~2天(tian)內。能度(du)過48小時(shi)(shi)以上者,一般可(ke)恢復健康而(er)留后(hou)遺(yi)癥(zheng)。死亡原(yuan)因主要是肺水腫(zhong)引起的嚴重缺氧及循環衰竭。晚期多半死于(yu)支氣(qi)(qi)(qi)管(guan)肺炎。

光(guang)氣(qi)有劇毒(du),是一種強刺激;刺激性(xing)氣(qi)體,具有爛干草氣(qi)味(wei)。吸入光(guang)氣(qi)引起肺水(shui)腫(zhong);肺炎等(deng),具有致死危險(xian)。

據長(chang)期人(ren)體(ti)吸(xi)入微量(liang)光(guang)氣(qi)(qi)實驗證(zheng)明:該(gai)氣(qi)(qi)體(ti)可使人(ren)體(ti)發胖,因此(ci)生產(chan)光(guang)氣(qi)(qi)的車間的工(gong)人(ren)可能出(chu)現(xian)發胖癥狀,與(yu)吸(xi)入的微量(liang)光(guang)氣(qi)(qi)有(you)關。

光氣毒劑

光(guang)氣是一種毒(du)害作用(yong)巨大的化學戰劑(ji),用(yong)于制造(zao)毒(du)氣彈。光(guang)氣毒(du)劑(ji)最早在一戰中應用(yong)。

自從伊(yi)普(pu)爾(er)毒(du)氣戰(zhan)后,戰(zhan)爭雙方的(de)決策者、指揮(hui)者開始熱衷化學戰(zhan)。戰(zhan)爭促進了(le)化學武器(qi)的(de)發展,一戰(zhan)中出(chu)現了(le)多種毒(du)劑,除了(le)氯(lv)氣外,又出(chu)現了(le)光氣,是(shi)殘害(hai)生靈的(de)戰(zhan)場毒(du)魔。

光(guang)氣是一氧化(hua)碳與(yu)氯氣在日光(guang)下合成,為無色(se)氣體,它能(neng)傷(shang)害人(ren)(ren)體呼(hu)吸器(qi)官,嚴重時導(dao)致人(ren)(ren)體死亡(wang)。1915年12月19日,德軍發射(she)裝填光(guang)氣的火箭彈。英軍陣地上有1000多人(ren)(ren)中毒,100多人(ren)(ren)死亡(wang)。

在一戰(zhan)中,光(guang)氣這(zhe)種(zhong)毒劑(ji)得到廣泛應用,交戰(zhan)雙方都使用了光(guang)氣這(zhe)種(zhong)毒劑(ji),使用量達到10萬噸之多。

二戰時,日軍(jun)鑒于芥子氣(qi)(qi)毒(du)(du)性(xing)過大(da),自己(ji)也無(wu)法防(fang)護(hu)。于是瞄準了光(guang)氣(qi)(qi),因為光(guang)氣(qi)(qi)只能(neng)通(tong)過呼吸道中毒(du)(du),而我(wo)軍(jun)又無(wu)防(fang)護(hu),所(suo)以大(da)量使用光(guang)氣(qi)(qi),并將其稱為“特種煙”(為日軍(jun)對窒(zhi)息性(xing)毒(du)(du)氣(qi)(qi)的隱(yin)敝(bi)稱呼)
